Terms and conditions governing the test report issued

1. The reported results are for information and interpretation on the referring doctor or such other medical professionals who understand reporting units, reference ranges and limitations of technologies Sigma Diagnostic will not be responsible for any interpretation what soever.
2. It is presumed that the lest(s) performed are, on the specimen(s) sample(s) belonging to the patient named or identified and the verifications of the particulars have been carried out by the patient or his/her representative at the generation point of said specimen. The reported results are restricted to the given speciment(s) only.
3. The test report shall not be reproduced except in full, without the written approval of Sigma Diagnostic. The test report in full or part shall not be used by client or be authorised by a client for promotional or publicity purposes In anyway that may consider to be misleading.
4. Results may vary from lab to lab and from time to time for the same parameter for the same patient. Assays are performed in accordance with standard procedures. The reported results are dependent on individual assay methods/equipments used and quality of specimen(s) received. All investigations have their limitations and isolated laboratory investigations may not confirm the final diagnosis of disease. They only help in arriving at diagnosis in conjunction with clinical presentation and related investigations. reports are to be interpreted, taking into consideration the above mentioned facts.
5. If the results are uinexpected or do not correlate clinically, kindly contact the laboratory for reconfirmation to Rule out any human error/. A repeat investigation may be entertained with 24-48 hrs from the time of testing on request by the client.
6. Histopathology specimen(s) will be preserved for one month from the date of testing whereas slides/reports for 3 month and FNAC slides for 5 days only. Or the Referring Doctor which could be free of cost if the basis for retesting is logical and for genuine reason. Other clinical specimen(s) will be retained according to their retention periods unless otherwise specified by the client during the sample registration requesting preservation for longer period. Such preservation shall be subject to sample integrity/stability and on payment of additional fee for the same.
7. Neither Sigma Diagnostics nor its directors/employees/representatives assume any loss or damage that may be incurred by any person including patient, as a result of assuming the meanings or contnts of the report.
8. This report is not valied for medico legal purposes.
9. Sigma Diagnostic will not take any responsibility for the sample integrity in particular with respect to international samples where government approvals and clearances are involved.

17. Values out of reference range requires reconfirmation before starting any medical treatment(s).
